Strategies for Increasing Medical Homes for Those in Need
Opportunity
The percentage of adults in Lincoln who do not have medical insurance more than doubled between 2005 and 2008, increasing from seven percent to 15 percent. Those who did not seek care due to cost also increased from seven percent to 15 percent; and, those who reported that they have one person who they view as their personal doctor or health care provider (a medical home) declined from 90 percent to 82 percent. Community focus groups show that these “safety net clients” have little or no understanding of the medical home concept.

Initiative
With this project, Healthy Lincoln will increase the number of safety net clients who establish and maintain medical homes; expand collaboration between safety net providers in the community; and increase the number of primary care physicians who serve as medical homes for safety net clients. A database will track all safety net clients and their integration into medical homes.

Key Goals and Objectives
The goal is for 75 percent of safety net clients to be assigned to medical homes, and for 50 percent of them to become established patients.
